Insurance Claims and Authorization Forms

Direction to Pay Authorization

When you submit a Direction to Pay form or any similar authorization document through our website, you are authorizing Exclusive Collision & Repair to act as your designated representative in communicating with your insurance carrier regarding the repair of your vehicle.

By submitting this form, you represent and warrant that:

  • You are the registered owner of the vehicle or have legal authority to authorize repairs on behalf of the owner

  • The information you provide is accurate and complete

  • You have the authority to direct payment to Exclusive Collision & Repair on your behalf

Insurance Claim AssistancE

We assist customers in navigating the insurance claims process, communicating with insurance adjusters, and pursuing fair compensation for covered repairs. This assistance is provided as a service to our customers and does not constitute legal representation or insurance advice.

We cannot guarantee specific claim outcomes, settlement amounts, or insurer approval of any repair scope. Final decisions regarding claims remain with your insurance carrier.

Customer Responsibility

You remain responsible for:

  • Paying any applicable deductible as required by your insurance policy

  • Providing accurate insurance and vehicle information

  • Communicating any changes to your claim or coverage in a timely manner

  • Understanding the terms of your own insurance policy

Repairs and Workmanship

Authorization Required

No repair work will begin on your vehicle without a signed repair authorization. Once authorized, we will perform the agreed-upon work using qualified technicians and appropriate materials.

Supplement and Change Orders

During the repair process, additional damage may be discovered that was not visible at the time of the original estimate. In such cases, we will notify you and your insurer of any supplements or changes in scope before proceeding with additional work.

Warranties

Any warranty terms applicable to your specific repair will be provided in writing at the time of your service agreement. Warranty coverage, duration, and exclusions vary by repair type and will be clearly communicated prior to vehicle drop-off.

We do not make warranties beyond those expressly stated in your written repair agreement. Nothing in these Terms creates any warranty obligation beyond what is expressly stated in that document.

Vehicle StoragE

If your vehicle is left on our premises after you have been notified that repairs are complete and you have failed to retrieve it within a reasonable timeframe, storage fees may apply in accordance with New York State law and our standard shop policies. We will provide notice before storage fees begin to accrue.

Towing ServicE

24-hour towing services are provided as an accommodation to customers and are subject to availability. Towing fees, if any, will be communicated before service is dispatched. By requesting towing, you represent that you have the authority to authorize the tow of the vehicle in question.

We are not responsible for pre-existing damage to a vehicle that is towed, and a condition report will be completed at the time of vehicle intake.

Limitation of Liability

To the fullest extent permitted by applicable law, Exclusive Collision & Repair shall not be liable for:

  • Any indirect, incidental, special, or consequential damages arising from your use of this website or our services

  • Delays caused by supply chain disruptions, insurer disputes, parts availability, or other factors outside our reasonable control

  • Loss or damage to personal property left inside a vehicle during the repair process

  • Any reliance you place on information published on this website that has since changed
